That's what I do, inject into the muscle I plan on training that day.

I concentrate on upper chest followed by flat work. Rarely will I do decline stuff. If I do, it's only for variety's sake.

It really depends for me. Sometimes I'll take a few weeks off or a few months. The biggest factor is whether or not my goodies drawer is full or not.
